229 - Hidden Hazards Underground: What the ICBM Missileer Community Teaches Us About Occupational Health

Session Description: Underground nuclear missile facilities represent one of the most extreme occupational environments in the modern era. Service members assigned to the U.S. Air Force Intercontinental Ballistic Missile (ICBM) mission spent decades working in confined launch control centers and missile alert facilities, often for 24–72 hour shifts, exposed to an array of toxic hazards: polychlorinated biphenyls (PCBs), radon, asbestos, diesel leaks, cryptographic material burn-off, and other poorly documented exposures.

In recent years, missile community veterans have raised alarms over disproportionate cancer diagnoses. Self-reported data now include over 800 cases of cancer and related illnesses. Many were diagnosed decades earlier than the national median age for their disease, and numerous families have been devastated by the loss of young officers in their 30s and 40s.

This session will use the ICBM community as a case study in occupational health methodology and how occupational health professionals can partner with scientists, public health leaders, and legal advocates to drive systemic change.

Healthy world: The proposed session, Hidden Hazards Underground: What the ICBM Missileer Community Teaches Us About Occupational Health, supports the spirit of the AOHC conference by showing how occupational and environmental medicine can transform lived experience into evidence, messaging, and systemic change. Occupational health professionals often encounter worker populations where exposures are unique, records are incomplete, and denominators are absent. This session addresses those challenges directly, offering a framework for transforming narrative into action.

The missileer community provides a compelling case study. For decades, service members operated in underground facilities with exposures to polychlorinated biphenyls, asbestos, radon, diesel fuel, and cryptographic material burn-off. Monitoring was sparse, documentation inconsistent, and risks underappreciated. Today, more than 800 self-reported cancer cases—many diagnosed decades earlier than expected—have galvanized veterans and families to demand answers. Their testimony raises critical questions: How do we honor worker voices? How do we responsibly analyze registry data without denominators? And how do we communicate uncertain findings to both affected populations and policymakers?

This session brings together experts across disciplines to explore those questions. Participants will hear the human narratives that first signaled a problem, review environmental and toxicologic evidence, consider statistical approaches for incomplete data, and learn risk communication strategies that balance urgency with credibility. The session will conclude with lessons from legal advocacy, highlighting how one family secured recognition of service-connected cancer, and broadening to policy reform efforts, including the push to extend presumptive toxic exposure recognition under frameworks like the PACT Act.

By moving from story to science to systems, the session reflects AOHC’s mission: to equip occupational health professionals with the tools to protect vulnerable populations in complex environments. The lessons extend well beyond missile fields—to firefighters exposed to PFAS, agricultural workers facing pesticides, or health care workers confronting novel pathogens. This session demonstrates how occupational medicine must listen, synthesize, and act, ensuring that worker narratives are never lost, but instead become the foundation for prevention, advocacy, and justice.
